Form A- Evaluation Selection and Waiver

Used at the beginning of the school year to select evaluator. *Updated: 8/14/15

Form A1: Change of Evaluator Document

This form is to be used if you are requesting (with reason) to change your evaluator

Form B- Initial Observation and Goal Setting

Filled in by teacher after first evaluation to record strengths and weaknesses and goals for the school year. To be shared with evaluator prior to first post-conference.

Form C- Teacher Observation Rubric

Evaluator uses for each observation (live or video)

Form D- Teacher Report Card

Final scores reported to give teacher rating for the year. Filled in by the evaluator.

Form P: Action Plan Form

This form is used from the end of your last evaluation to the beginning of your next year's evaluation *updated: 8/14/15

Form R- Rebuttal Form

Form submitted to refute a score given by evaluator. Note: documentation will need to be provided to support claim

Form V- Video Submission

Submit with every video.
